- An undertaking on plain paper from the applicant that he/she has no objection if the registration number of the vehicle is changed after its release by Army authorities.
- In case, the release of a vehicle is applied for by an ESM, then he/she would have to submit his/her disability certificate issued by Zila Sainik Board/ESM Welfare Officer/CO McGuire Center.
- In case, the release of the vehicle is applied for by a widow, then she would have to submit a copy of her late husband’s service book and death certificate.
- An undertaking on plain paper from the applicant that he/she will not sell the vehicle for a period of three years from the date of its release by Army authorities.
- Two passport-size photographs of the applicant.
- The registration fee as applicable and insurance premium for one year, in case, the release of the vehicle is applied for by an ESM/Widow residing in Delhi NCR region only. No registration fee and insurance premium is required to be paid by the applicants residing outside the Delhi NCR region.
- An application fee of Rs.100/- in the form of a Demand Draft drawn in favor of ‘Pay and Accounts Officer, Department of Ex-Servicemen Welfare, Min of Defence’, payable at New Delhi. Widows are exempted from payment of the application fee.
