Top 10 The Scariest Flight Stories in Aviation History

Few experiences can be more terrifying than discovering, thousands of feet above the ground, that something has gone catastrophically wrong with the aircraft carrying you.

Yet aviation history contains stories that sound almost impossible: an aircraft losing all engine power over the Atlantic Ocean, a passenger jet losing a huge section of its fuselage at cruising altitude, an airliner becoming almost impossible to control after losing all three hydraulic systems, and a giant Airbus A380 suffering an engine explosion that damaged systems throughout the aircraft.

Some of these flights ended in tragedy. Others became extraordinary stories of survival. Almost all changed aviation.

The following ten stories are not ranked simply by the number of people who died. They are ranked by the sheer terror of the circumstances, the extraordinary challenges faced by crews and passengers, and the lessons investigators ultimately uncovered.

10. British Airways Flight 5390 — The Captain Who Was Nearly Sucked Out of the Cockpit

On June 10, 1990, British Airways Flight 5390 was flying from Birmingham to Málaga aboard a BAC One-Eleven.

Nothing about the flight initially suggested that it would become one of aviation’s strangest survival stories.

Then, at approximately 17,300 feet, one of the cockpit windscreen panels suddenly separated from the aircraft.

The result was an explosive decompression.

The captain was pulled partly through the opening.

The situation sounds almost unbelievable: an airliner was continuing to fly while its captain was trapped partly outside the cockpit in freezing air and enormous wind pressure.

The first officer, Alastair Atchison, suddenly had to control the aircraft and deal with an emergency that was unlike anything most pilots could reasonably expect to experience.

The cabin crew also had to act quickly. Members of the crew managed to restrain the captain while the aircraft descended toward Southampton.

Remarkably, all 87 people aboard survived. The captain suffered serious injuries, but the accident became a powerful lesson in aircraft maintenance and human resilience. The investigation found that the windscreen had been installed with incorrect fasteners, and the fasteners were not the correct size for the job.

What makes Flight 5390 especially frightening is its almost surreal quality. A routine component that passengers would never think about—the cockpit windscreen—suddenly became the central threat to the aircraft.

The accident demonstrated a fundamental truth about aviation: catastrophic accidents do not always begin with dramatic mechanical failures. Sometimes they begin with something as apparently mundane as an incorrectly installed part.

9. Aloha Airlines Flight 243 — When the Roof of the Airplane Disappeared

Aloha Airlines Flight 243 was flying between Hilo and Honolulu, Hawaii, on April 28, 1988.

The Boeing 737 was cruising at approximately 24,000 feet when disaster struck.

An approximately 18-foot section of the upper fuselage suddenly separated from the aircraft.

Passengers were suddenly exposed to the outside atmosphere.

One flight attendant was swept from the airplane and was presumed to have been fatally injured. Seven passengers and another flight attendant suffered serious injuries.

And yet the airplane remained controllable.

The pilots performed an emergency descent and diverted to Kahului Airport on Maui, where they landed safely. Of the 95 people aboard, one person died and eight suffered serious injuries.

The photographs of the aircraft after landing are among the most disturbing images in aviation history: a Boeing 737 with a huge portion of its forward upper fuselage simply missing.

The terrifying part is that the airplane had not exploded in the conventional sense. Instead, years of structural fatigue had gradually weakened the fuselage.

The NTSB concluded that the probable cause was the failure of Aloha Airlines’ maintenance program to detect significant disbonding and fatigue damage in the fuselage lap joint.

The accident helped change the way regulators and airlines thought about aging aircraft.

It showed that metal fatigue could become a hidden enemy—one that develops slowly and silently until the structure finally gives way.

For the passengers aboard Flight 243, however, there was nothing theoretical about it. One moment they were sitting in an ordinary airliner; the next, part of the airplane around them was gone.

8. Air Transat Flight 236 — The Giant Airliner That Became a Glider

Few aviation emergencies are as frightening as running out of fuel.

Doing so over the Atlantic Ocean, at night, hundreds of kilometres from a suitable airport, is another matter entirely.

On August 24, 2001, Air Transat Flight 236 was flying from Toronto to Lisbon aboard an Airbus A330 with 306 people aboard.

During the flight, the aircraft developed a fuel leak.

The problem grew progressively worse.

Eventually, both engines lost power.

The Airbus A330—an aircraft designed for long-range operations with powerful turbofan engines—became, in effect, a giant glider.

The crew now had to keep the aircraft airborne without engine thrust and find somewhere to land.

The pilots ultimately diverted toward Lajes Airport in the Azores.

They were able to glide the aircraft toward the island and make an emergency landing at Lajes Air Base.

All 306 people survived, although 18 people were injured.

The investigation found that a fuel leak had developed following the installation of an incorrect hydraulic component during maintenance. The configuration allowed a hydraulic line and fuel line to come into contact, eventually causing the fuel line to rupture.

There is something uniquely terrifying about this story.

An aircraft with hundreds of people aboard was flying through the darkness over the ocean, while its fuel supply was disappearing.

Then the engines stopped.

The pilots had no opportunity to simply restart them. Their only option was to turn the aircraft into a glider and find land.

The successful landing became one of the great examples of crew resource management and emergency airmanship.

7. Singapore Airlines Flight 006 — Taking Off on a Runway That Wasn’t Supposed to Be Used

Singapore Airlines Flight 006 presents a different kind of aviation nightmare.

Sometimes the terrifying event isn’t a mechanical failure.

It is realizing too late that the aircraft is in the wrong place.

On October 31, 2000, Singapore Airlines Flight 006, a Boeing 747, was preparing to depart Taipei during bad weather.

The aircraft began its takeoff from the wrong runway.

The runway was partially closed because of construction.

The crew did not recognize their position before beginning the takeoff roll.

At approximately 23:17 local time, the aircraft struck construction equipment while accelerating for takeoff. The Boeing 747 crashed and broke apart.

There were 179 people aboard; 83 were killed.

The investigation found that the crew had lost situational awareness and had not adequately verified that they were aligned with the correct runway. Investigators also identified weaknesses in procedures that could have required positive confirmation of the active runway before takeoff.

This accident is frightening precisely because nothing had to physically fail on the airplane.

The aircraft was mechanically capable of flying.

The engines worked.

The controls worked.

The airplane was simply taking off from the wrong piece of pavement.

It demonstrated how aviation safety depends not only on technology, but on human perception, communication, procedures and the ability to recognize when something does not look right.

6. United Airlines Flight 232 — The Airliner That Lost All Three Hydraulic Systems

On July 19, 1989, United Airlines Flight 232 was cruising toward Chicago aboard a McDonnell Douglas DC-10.

Then the No. 2 tail-mounted engine suffered a catastrophic failure.

Fragments from the engine’s fan assembly penetrated the aircraft.

The consequences were extraordinary.

The DC-10 had three independent hydraulic systems designed to provide redundancy for the aircraft’s flight controls.

The engine failure damaged all three.

The pilots suddenly found themselves flying an airplane whose normal flight-control system was essentially unavailable.

The aircraft became extraordinarily difficult to control.

The crew improvised an unprecedented method of controlling the airplane using differential thrust from the engines.

They were eventually able to reach Sioux Gateway Airport in Iowa.

The landing was catastrophic. The airplane crashed during the attempted landing and broke apart.

Of the 296 people aboard, 111 died. But 185 survived—an extraordinary survival rate considering the nature of the failure.

The accident became one of the most important examples of crew resource management in aviation history.

The captain, first officer, flight engineer and other crew members worked with an instructor pilot who happened to be aboard the aircraft. Ground personnel and emergency responders also played crucial roles.

The NTSB determined that the accident was caused by the failure of the No. 2 engine fan disk following a fatigue crack that originated from a metallurgical defect.

Flight 232 is terrifying because it illustrates how quickly redundancy can disappear.

An airplane can have three supposedly independent hydraulic systems.

But a single failure can sometimes affect all three.

The story also demonstrates something equally important: when conventional systems fail, trained humans can sometimes improvise solutions that engineers never intended to be used in normal operations.

5. Qantas Flight 32 — The A380 With an Engine That Exploded

The Airbus A380 is one of the largest passenger aircraft ever built.

It has four engines.

That redundancy is reassuring—until one engine fails catastrophically.

On November 4, 2010, Qantas Flight 32 departed Singapore for Sydney with 469 people aboard.

Only minutes after takeoff, the crew heard two enormous bangs.

The No. 2 engine had suffered an uncontained failure.

Engine debris penetrated areas of the aircraft, damaging multiple systems.

The resulting damage affected electrical systems, flight controls, fuel systems and other components.

The aircraft was still airborne, but the crew had an extraordinary number of failures to manage.

The captain and his crew worked through a long series of warnings and system problems while trying to determine what remained functional.

The ATSB investigation documented the enormous workload created by the engine failure and its consequences. The aircraft eventually returned to Singapore and landed safely. All 469 people aboard survived.

Qantas Flight 32 is particularly remarkable because the aircraft did not simply suffer an engine failure.

It suffered a chain reaction of system damage.

The challenge for the crew was therefore not simply:

“Can we fly with one engine out?”

It was:

“What parts of this enormous aircraft still work, and how can we safely land it?”

The flight became an important case study in systems engineering, crew coordination and decision-making under extreme pressure.

4. Alaska Airlines Flight 261 — Fighting an Airplane That Wanted to Dive

On January 31, 2000, Alaska Airlines Flight 261 was flying from Puerto Vallarta toward Seattle with a planned stop at San Francisco.

The aircraft was a McDonnell Douglas MD-83.

During the flight, the pilots experienced problems with the horizontal stabilizer trim system.

The situation deteriorated.

The aircraft eventually entered an extreme nose-down attitude.

The pilots fought desperately to regain control.

At one point they managed to recover from an inverted or near-inverted attitude, but the problem was not solved.

The stabilizer system ultimately failed catastrophically.

Flight 261 crashed into the Pacific Ocean near California.

All 88 people aboard—the two pilots, three cabin crew and 83 passengers—were killed.

The NTSB investigation focused heavily on the jackscrew assembly, lubrication and inspection practices, maintenance intervals, and the design of the MD-80 stabilizer trim system.

What makes Flight 261 so frightening is the struggle.

The pilots knew they had a serious control problem.

They attempted different procedures to recover the aircraft.

They communicated with air traffic control and with other aircraft.

And yet the mechanical problem continued to overpower their efforts.

For investigators, the accident became an important warning about the consequences of maintenance failures in safety-critical systems.

For the people aboard Flight 261, it was a battle against a mechanical system they could not ultimately overcome.

3. Air France Flight 447 — Disappearing Into the Atlantic

Air France Flight 447 was flying from Rio de Janeiro to Paris on June 1, 2009.

The Airbus A330 was crossing the Atlantic at night.

There were 228 people aboard.

The flight entered an area of intense tropical weather.

Then the aircraft’s airspeed measurements became unreliable, likely because ice crystals temporarily obstructed the Pitot tubes.

The autopilot disconnected.

The flight-control system changed modes.

And the pilots suddenly had to fly the aircraft with unreliable airspeed information in darkness over the ocean.

The situation quickly deteriorated.

The aircraft entered a high-altitude aerodynamic stall and remained in the stall until it struck the Atlantic Ocean.

All 228 people aboard died.

The accident became one of the most extensively studied aviation disasters of the modern era.

The BEA investigation found that the initial technical problem was only the beginning. Human factors, the crew’s understanding of the situation, conflicting indications, the response to the stall and cockpit coordination all became part of the accident sequence.

The most unsettling element is that the airplane was not destroyed by a massive explosion or structural breakup.

It was still essentially an intact aircraft.

The tragedy arose from a combination of unreliable information, automation changes, human reactions and failure to recognize the aircraft’s actual aerodynamic state.

The accident became a landmark case in the study of automation, pilot training, surprise and loss of control.

It also demonstrated one of aviation’s most dangerous realities:

An aircraft can be technically capable of flying while the humans controlling it no longer understand what the aircraft is actually doing.

2. Japan Airlines Flight 123 — Thirty-Two Minutes of Fighting a Broken Aircraft

Japan Airlines Flight 123 is one of the most horrifying stories in aviation history.

On August 12, 1985, a Boeing 747 departed Tokyo for Osaka.

Approximately twelve minutes after takeoff, the aircraft suffered a catastrophic structural failure.

The rear pressure bulkhead failed.

The resulting decompression caused enormous structural damage, including the loss of the vertical stabilizer and damage to the aircraft’s hydraulic control systems.

The pilots were suddenly flying a crippled Boeing 747 with severely compromised control capability.

Yet they did not immediately lose the aircraft.

For approximately half an hour, the crew fought to keep the airplane airborne.

They attempted to control its pitch and direction using combinations of engine thrust and the limited control authority that remained available.

The airplane oscillated dramatically as the pilots struggled to keep it away from mountainous terrain.

Eventually, Flight 123 crashed into Mount Takamagahara in Gunma Prefecture.

Of the 524 people aboard, 520 died. Four survived.

The investigation traced the structural failure back to damage sustained during an earlier tailstrike incident and an improper repair of the rear pressure bulkhead. The subsequent fatigue failure ultimately produced the catastrophic structural sequence.

What makes Flight 123 particularly horrifying is the length of the struggle.

This was not a sudden event followed immediately by impact.

The crew had time to understand that something was terribly wrong.

They had time to fight the aircraft.

And the aircraft remained in the air for an extended period despite suffering damage that would normally be considered catastrophic.

The accident remains a devastating reminder that a seemingly successful repair can become the starting point for a disaster years later.

1. US Airways Flight 1549 — The Miracle on the Hudson

The number-one story on this list is unusual because it is terrifying without becoming a mass-fatality disaster.

On January 15, 2009, US Airways Flight 1549 departed New York’s LaGuardia Airport for Charlotte.

The Airbus A320 climbed normally.

Then, only about two minutes after takeoff, it encountered a flock of large birds.

The birds were ingested into both engines.

Both engines suffered an almost complete loss of thrust.

The aircraft was only a few thousand feet above the ground.

There was no time to return to the airport in the conventional sense.

The crew considered their options.

The airplane was losing altitude.

New York City was directly below.

Captain Chesley “Sully” Sullenberger and First Officer Jeffrey Skiles determined that they could not safely reach an airport.

They chose the Hudson River.

The Airbus A320 descended toward one of the busiest metropolitan areas in the United States and landed on the river.

The aircraft stopped on the water.

Passengers evacuated onto the wings and into emergency rafts while nearby ferries and emergency crews rushed to the scene.

All 155 people aboard survived, although several people suffered serious injuries.

The NTSB determined that the probable cause was the ingestion of large birds into both engines, resulting in the near-total loss of thrust and the subsequent ditching.

The FAA later described the event as extraordinary because of the simultaneous loss of thrust in both engines immediately after takeoff and the successful emergency water landing without loss of life.

What makes Flight 1549 the most frightening—and simultaneously most inspiring—story on this list is the tiny amount of time available for decision-making.

There was no runway ahead.

There was no comfortable altitude from which to troubleshoot.

There was no time to wait for another solution.

The crew had to make an irreversible decision almost immediately.

And it worked.

What These Ten Flights Tell Us About Aviation

These stories may sound like arguments against flying.

In reality, they tell a very different story.

Modern aviation safety has been built partly from the painful lessons of accidents like these.

Aloha Airlines Flight 243 highlighted the danger of metal fatigue and inadequate inspection.

Japan Airlines Flight 123 demonstrated the catastrophic consequences that can follow an improper structural repair.

United Airlines Flight 232 exposed weaknesses in the assumed independence of redundant systems.

Alaska Airlines Flight 261 showed the importance of maintenance and inspection of critical flight-control components.

Air France Flight 447 changed thinking about pilot training, automation, unreliable airspeed and stall recovery.
